Pay/Employment Update for Employees

Authorized individuals ONLY should use this form to submit for additional pay, a bonus, a pay increase/decrease, or employment separation. This form is for things such as additional pay, bonuses, pay changes, retirement or medical deductions, and changes in employment.

***PLEASE NOTE*** THIS FORM SHOULD NOT BE USED FOR INDEPENDENT CONTRACTORS OR ORGANIZATIONS, please submit the recurring payment update form for all changes to recurring payments to contractors or organizations.

Submission Form

All pay changes that will be effective for the first paycheck of the new year must be received no later than 12/15. This does not apply to separations. Thank you!
